Insurance information

I do not accept any insurance for the following reasons:

I provide a Superbill (Receipt) that patients can submit to their insurance company for partial reimbursement or to be applied to their deductible. This usually involves the patient faxing in the Superbill to a specific number at your insurance. They may require another form to be filled out in addition to the Superbill. Call your insurance for this information. We do not have the staff to do provide this service.

Dealing with insurance companies distracts me and my staff from providing care for our patients.

Insurances no longer provide adequate reimbursements to psychiatrists without having to see large volumes of patients in short periods of time.

Before your scheduling your appointment, please access the patient forms below. Please print, complete, and fax the forms to 1-888-972-3955, then bring the forms to your appointment. Initial evaluations will be scheduled once a $150 deposit is paid that goes toward the visit. Please bring in your Photo ID and be prepared to supply a urine sample.
